I would like Polk-Burnett Propane Services to install a monitor on the propane tank at my property.

I would like to receive the smartphone app information so I can have access to the monitor readings.

Delivery Service

Polk-Burnett will schedule a delivery when the monitor shows tank to be 20-30% (account must be current).

I, the customer, will call to schedule a delivery as needed.

The monitor remains property of Polk-Burnett Propane Services, Inc. and is not to be removed or altered by anyone other than a Polk-Burnett Service Technician.

If you no longer wish to use it and it is not returned to us, it could be subject to a $150 replacement fee. Polk-Burnett Propane Services, Inc. is not responsible for equipment malfunctions.

I understand that the monitor is $6/mth billed annually in May or prorated once monitor has been installed.

I agree to the tank monitor terms listed above.
